      • Amazing team support and empathetic leadershipTake your nursing career to a new level of caring.As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating performance and making necessary changes to increase productivity and quality of care.
        • Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
          • Identify patient's physical, psychosocial, and environmental needs, and reassess as needed.
            • Initiate and coordinate the plan of care with accurate clinical notes.
              • Document problems, goals, interventions, and patient response to hospice care.
                • Collaborate with patient, family, physician, and IDG to provide patient and family care daily.
                  • One year of clinical experience is required.
                    • Current RN licensure in the state of practice and one year of clinical experience are required.
                    • Current CPR Certification is required.
                    • A current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation are required.
